- Job Description
Responsible for initiation and completion of all Minimum Data Set (MDS) Assessments of residents on admission, quarterly, annually, and as changes in condition / status occur, by the Resident Assessment Instrument (RAI) set forth by HCFA. Responsible for the preparation / revision of the Interdisciplinary Plan of care for each resident based on their individual needs and information triggered from the MDS. Assists with other programs and serves on other committees within the Nursing Home as able. Maintains status as an active Registered Nurse.

Education: Associate Degree in Nursing or higher
Licensed: Registered Nurse
Experience: Preferred but not required
